Sumter Convenient Care offers complete family health care services to the community. Our outpatient office is designed to serve you and your family’s healthcare needs in a friendly, comfortable setting. We provide the best medical care to every patient every day. With two dedicated full time Physicians and a team of highly qualified healthcare professionals, the staff at Sumter Convenient Care is there when you need them.

We see patients on an appointment basis. Please call for an appointment. We do our best to accommodate walk-ins and same day appointments for urgent care needs.

SWGHC provides access to health care for people, who might not otherwise be able to afford it. Health center services are not free. However, to ensure that income or lack of insurance is not a barrier to care, patients who are not covered by public or private insurance are charged on a sliding fee scale according to income. Co-payments are due at the time of the office visit. For patients who qualify for our sliding fee program, the patient’s percentage is due at the time of service. All our centers accept Visa, Master Card, Discover, Personal Checks (with identification) and Cash.

Sliding Scale. Patients who wish to participate in our sliding-fee-program are encouraged to ask for a sliding fee application form, which includes verification of income. In order for a patient to qualify for the sliding fee program, the information must be provided at the time of the visit. If the qualifying documentation is not provided, the patient will be responsible for 100% of the cost for services rendered.

 

This is a health care center funded by the federal government. This means even if you have no insurance you can be covered. The center is also income based for those making an income. This health center can cover services such as checkups, treatment, pregnancy care (where applicable), immunizations and child care (where applicable), prescription medicine and mental and substance abuse where applicable.
